Certified Nursing Assistant - Hospice
About The Position
The Certified Nursing Assistant (CNA) provides resident care under the direction of a Case Manager/DON. The CNA follows resident assessments and care plans, as well as instructions from nursing management. They work with licensed care staff to coordinate appropriate care and follow Company nursing policies, procedures, and safety guidelines. The role involves working one-on-one with patients while collaborating with a broader patient care team. The position requires the ability to work independently and to drive, with reliable transportation, a valid driver’s license, and auto insurance.
Requirements
- Hold Certified Nursing Assistant (CNA) certification in the state or states of practice.
- Be at least 18 years of age.
- Have a valid driver’s license, reliable transportation, and auto insurance, and be willing to drive.
- Be able to work independently and as part of a team.
- Demonstrate a commitment to improving quality of life and partnering with community organizations in patient care.

- Eagerness to pursue certifications and further education.
Responsibilities
- Provide resident care in accordance with resident assessments and care plans, and as directed by nursing management.
- Coordinate appropriate care with other licensed care staff.

- Collaborate with nurses, social workers, chaplains, volunteers, and STNA leadership.
