About The Position
Polaris Rehabilitation & Care Center is seeking a dependable, compassionate PRN CNA Transportation Aide to help residents travel safely to medical appointments and other approved destinations. This as-needed role combines resident care, transportation, and meaningful resident interaction. Hours depend on resident appointments and facility needs; weekday availability is preferred, and hours are not guaranteed. The role may also provide additional CNA support when transportation assignments permit. Applicants must be able to perform essential duties, including driving, resident transfers, and wheelchair assistance, with or without reasonable accommodation. Requirements
- Active Wyoming CNA certification in good standing.
- Valid driver’s license, acceptable driving record, and ability to meet facility vehicle-insurance requirements.
- Safe transfer and mobility-assistance skills.
- Dependability, patience, and strong communication.
- Ability to complete required background checks and training.
- Ability to perform essential duties, including driving, resident transfers, and wheelchair assistance, with or without reasonable accommodation.
Nice To Haves
- Transportation or wheelchair-accessible vehicle experience.
- Weekday availability preferred.
Responsibilities
- Safely drive residents to appointments and other approved destinations.
- Assist with transfers, wheelchair securement, mobility, and personal care within CNA scope.
- Coordinate appointment schedules and paperwork with Nursing.
- Monitor resident comfort and promptly report changes or concerns.
- Complete transportation logs, vehicle safety checks, and routine vehicle cleaning.
- Provide additional CNA support when transportation assignments permit.
