About The Position

A Certified Nursing Assistant performs nursing procedures and patient care tasks as outlined by a patient’s plan of care. This position is full-time, nonexempt, with standard hours of 6am to 10am, Monday to Friday. It also includes participation in holiday and weekend on-call rotations with hours of 7am to 3:30pm or 10:30am to 7pm.

Requirements

• Must have current, valid Wisconsin Nursing Aid certification
• Requires, a valid driver's license, vehicle with auto coverage
• Satisfactory completion of a caregiver and criminal background and drug screening

Nice To Haves

• High School Graduate preferred
• Previous Hospice/Palliative experience preferred

Responsibilities

• Administers patient care under the direction of the RN and as instructed by the CNA care plan
• Reports on changes in the client’s condition and/or needs to the RN Case Manager
• Provides personal care, including but not limited to bathing, shampooing, skin and nail care, oral hygiene, shaving, dressing.
• Performs household tasks essential to health care, including but not limited to meal preparation and feeding, laundry, light housekeeping.
• Accurately documents the provided cares and submits the record of care provided and pertinent observations following Hospice Alliance policy and procedure.
• Attends CNA in-services to ensure compliance with the state requirement of 12 continuous education hours per year.
• Communicate effectively with all members of the interdisciplinary team.
• Other duties as assigned.
