About The Position

The Certified Nursing Assistant (CNA) works under the direction of a Case Manager/DON and provides care to assigned residents in accordance with resident assessments, care plans, and nursing-management direction. The CNA collaborates with licensed care staff to coordinate appropriate care and follows company nursing policies, procedures, and safety guidelines. The role involves working one-to-one with patients while receiving support from the patient-care team, and contributing to Suncrest Hospice’s goal of providing exceptional care and service to patients and families.

Requirements

• Certified Nursing Assistant (CNA) certification in the state or states of practice.
• Must be at least 18 years of age.
• Must be willing to drive and have reliable transportation, a valid driver’s license, and auto insurance.
• Able to work independently on a day-to-day basis.
• Ability to work well within a team environment.
• Strong commitment to improving quality of life while partnering with community organizations in patient care.

Responsibilities

• Provide care to assigned residents in accordance with assessments, care plans, and direction from nursing management.
• Coordinate appropriate resident care with other licensed care staff.
• Follow company nursing policies and procedures and safety guidelines.
• Collaborate with nurses, social workers, chaplains, volunteers, and STNA leadership in patient care.
