About The Position

Moments Hospice is hiring a full-time, field-based Certified Nursing Assistant (CNA) / Hospice Aide to provide hands-on personal care to hospice patients wherever they call home, including private residences, assisted living communities, and care facilities. The role supports patients’ comfort and dignity and provides reassurance to their families.

The CNA works closely with a dedicated RN Case Manager as part of an interdisciplinary care team. The position includes structured training and the opportunity to build skills in a collaborative team environment. The schedule is Monday through Friday, 8:30 AM to 5:00 PM. Pay is $18–$23 per hour.

A fleet car is provided for business and personal use, with fuel and insurance covered at minimal cost, based on eligibility. A company cell phone is also provided.

Requirements

• Active Certified Nursing Assistant (CNA) certification.
• Ability to work in an electronic medical record system.
• Strong written, verbal, and active-listening communication skills.
• Excellent customer service, problem-solving, and time-management skills.
• Compassion, patience, and ability to handle emotional situations with care.
• Strong understanding of professional boundaries.
• Ability to lift 50 lbs. and a good understanding of body mechanics.
• Valid driver's license and current auto insurance.

Nice To Haves

• Healthcare caregiving experience in home care, assisted living or memory care, skilled nursing, a hospital, or hospice.
• Current CPR certification.

Responsibilities

• Provide personal care and assistance with activities of daily living, including bathing, dressing, grooming, toileting, and skin care.
• Assist patients with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient's plan of care under the direction of the RN Case Manager.
• Provide comfort and companionship to patients and support to their families.
• Maintain electronic medical records.
