About The Position

Princeton Transitional Care & Assisted Living, a partnership between Signature HealthCARE and Ballad Health, provides skilled-nursing care, assisted living, and short-term skilled-nursing and rehabilitation services in the Tri-Cities area. Signature HealthCARE is a family-based healthcare company providing integrated services across multiple states, including skilled nursing, rehabilitation, assisted and memory care, and home-based services. The Certified Nursing Assistant provides direct resident care under the supervision of licensed nursing personnel and helps maintain a compassionate physical and psychosocial environment in accordance with facility policies and procedures. The role supports residents’ activities of daily living, rights, and choices, and contributes to a safe, clean, and supportive care setting.

Requirements

Hold a current, active Nursing Assistant certification from the state in which the facility operates.
Have effective verbal and written communication skills.
Be able to work collaboratively within a nursing team and under the direction of licensed nursing staff.
Be customer-service oriented and able to work well under pressure.
Be approachable, flexible, and adaptable to change.
Be able to function independently and demonstrate flexibility, personal integrity, and the ability to work effectively with stakeholders and vendors.

Responsibilities

Perform direct resident care duties under the supervision of licensed nursing personnel.
Help promote a compassionate physical and psychosocial environment in accordance with facility policies and procedures.
Provide activities of daily living within the scope of practice and the resident’s plan of care.
Change bed linen and maintain appropriate grooming supplies in resident rooms.
Ambulate and transfer residents using appropriate assistive devices and body mechanics.
Record resident vital signs, weights and measurements, intake and output, food consumption, and other assigned information.
Turn and position residents as necessary.
Recognize and support resident rights and choices within the care environment.
Maintain established housekeeping standards within assigned duty areas.
Answer call lights promptly and assist with resident needs.
Assist in orienting and training other staff, as requested.
Complete other special projects and duties as assigned.
