About The Position

Prime Home Health is seeking compassionate, dependable Certified Nursing Assistants (CNAs) to provide exceptional in-home care to patients throughout the San Antonio area on a PRN basis. Help patients maintain their independence while delivering high-quality, one-on-one care in the comfort of their homes.

Requirements

• Current Texas CNA Certification
• Current CPR certification
• TB test taken within the past year
• High school diploma or equivalent
• Computer skills required (EHR experience preferred; Kinnser/WellSky, Slack a plus)
• Reliable transportation and valid driver's license
• Strong communication skills and a compassionate, patient-focused attitude

Nice To Haves

• 2 Years Home Health experience preferred
• EHR experience preferred; Kinnser/WellSky, Slack a plus

Responsibilities

• Assist with bathing, dressing, grooming, toileting, and mobility
• Monitor and document vital signs
• Provide medication reminders and assistance
• Prepare light meals and encourage proper nutrition
• Perform light housekeeping and errands
• Offer companionship and emotional support
• Document patient care accurately and timely in our EHR system
• Communicate changes in patient condition to the supervising nurse
