About The Position

A-Team Solutions is seeking six Certified Nursing Assistants (CNAs) for full-time positions at the VA in Menlo Park, CA. These roles involve day, evening, and night shifts. The primary focus is providing direct patient care and close observation, documenting patient behavior, activities of daily living, and physical care as directed by a Registered Nurse (RN). Documentation may be required as frequently as every 15 minutes. Responsibilities include assisting patients with personal hygiene, meals, mobility, and other nursing care needs in a hospital or long-term care setting under the supervision of an RN or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN). This includes taking vital signs, monitoring intake and output, Foley care, collecting specimens, and managing oxygen and suction equipment. CNAs will also communicate with patients, families, and staff, and may serve as preceptors for new CNAs. They must be able to recognize and respond to emergent patient situations, such as the need for basic life support or fall prevention, and report any changes in patient status immediately to the nurse.

Requirements

  • Valid/unrestricted CNA certification
  • BLS (American Heart)
  • Two years of experience as a CNA
  • Med/Surg experience
  • Received the COVID-19 vaccination

Responsibilities

  • Provide direct patient care and/or close observation of patients.
  • Document patient behavior, Activity of Daily Living, and physical care on observation flow sheets every tour, potentially every 15 minutes.
  • Deliver direct patient care including personal hygiene, assistance with meals, lifting and moving, and ambulation.
  • Assist in the full range of nursing care to patients/residents with physical and/or behavioral problems under the direction of an RN and/or LVN/LPN.
  • Take measures, records and reports weight, height, vital signs, intake and output, foley care, collecting urine, stool and sputum tests, set up and monitor wall and tank oxygen, continuous and intermittent suction, collects and assembles chest suction equipment.
  • Communicate orally with patients/residents, families, interdisciplinary team, and other personnel.
  • Serve as a preceptor to new CNAs by assisting with their orientation and overseeing/assessing their practical experience.
  • Recognize and react to emergent patient/resident care situations and intervene while waiting for assistance (e.g., basic life support, fall prevention, bleeding control, behavior crisis).
  • Report changes in mental or physical status immediately to assigned nurse/charge nurse.
