Requirements

• CNA certification required in the state hired to work, or an active LPN or RN license.
• Valid Driver’s License.
• Auto Insurance coverage.

Nice To Haves

• High school diploma preferred.
• One year home health aide or nursing assistant experience preferred.
• Use of technology-based tools, including electronic medical record, strongly preferred.

Responsibilities

• Delivery of personal care and minimal homemaker services (i.e. services promoting a safe and clean patient environment) to hospice patients in the home, assisted living facility, or long-term care facility.
• Services are provided based on Hosparus Health policies and according to the CNA plan of care (POC).
• Participates in the development of policies and procedures that guide the provision of hospice care.
