This position provides in-home services to patients, focusing on direct patient care as ordered by a physician and indicated by the plan of care. Responsibilities include assisting with daily living activities such as baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing, feeding, and transferring patients. The role also involves taking accurate vital signs, assisting with ambulation, positioning patients, and ensuring patient safety. The Hospice CNA participates in case conferences, communicates patient findings to licensed staff (Case Manager, Clinical Manager, or supervising RN), and maintains clear, concise documentation in the medical record. The role requires supporting cultural practices that do not interfere with medical treatment and participating in other agency-mandated activities.
