This full-time Hospice CNA position involves providing compassionate care to hospice patients, ensuring their comfort and dignity. Responsibilities include assisting with daily living activities, monitoring and documenting patient vital signs and changes in condition using EMR/EHR systems, collaborating with healthcare team members on care plans, adhering to HIPAA regulations, supporting families emotionally, and driving to patient locations as needed.

Requirements

1 years of CNA experience required
Licensed CNA in State of Indiana
Current driver's license
Must have reliable transportation and insurance

Nice To Haves

One year Hospice experience preferred

Responsibilities

Provide compassionate care to hospice patients, ensuring their comfort and dignity.
Assist with daily living activities such as bathing, dressing, and grooming.
Monitor and document patient vital signs and changes in condition using EMR/EHR systems.
Collaborate with healthcare team members to develop and implement care plans.
Adhere to HIPAA regulations and maintain confidentiality of patient information.
Support families by providing emotional support and education about hospice services.
Drive to patient locations as needed, ensuring timely and reliable service.
