About The Position

The Hospice CNA role involves providing direct patient care to hospice patients in various settings, including personal residences, assisted living facilities, and skilled nursing facilities. The typical schedule is Monday to Friday, 8 AM to 5 PM, with CNAs seeing 5-7 patients daily and spending 45-60 minutes with each. Responsibilities include a wide range of personal care tasks such as bathing, dressing, mouth care, assisting with toileting, incontinence care, repositioning, and meal assistance. Documentation is completed at the bedside using a company-provided cellphone. CNAs are also responsible for tidying the patient's immediate area, changing bed linens, maintaining adequate supplies in their vehicle, and picking up new supplies from the branch office. A significant part of the role involves driving between patient visits, with mileage being reimbursable. Close collaboration with RN case managers is required to communicate patient status changes and address patient or family concerns. Additionally, CNAs will train caregivers to provide personal care to their loved ones within the scope of practice and coordinate visit times with patients or caregivers.

Requirements

  • High school diploma or equivalent
  • Completion of CNA course
  • Must attend 12 hours of in-service training supervised by a Registered Nurse on a rolling 12-month basis
  • Competency of the CNA is assessed in the home every 14 days
  • An onsite evaluation occurs once a year
  • Has not had a lapse of 24 consecutive months where the aide has not provided services for compensation
  • Current certification with State Board of Nursing, or current registration with Department of Facilities Service in the State of North Carolina/ South Carolina
  • No prior experience required

Nice To Haves

  • Two years experience with hospice, home health or oncology preferred

Responsibilities

  • See patients wherever they consider home (personal residence, assisted living facilities, skilled nursing facilities)
  • Provide quality care regardless of circumstance to a variety of different socioeconomic and cultural backgrounds
  • Chart within the facility EMR if patients are in facilities
  • Care for patients and complete documentation at the bedside on a company provided cellphone
  • Provide direct patient care such as bathing, peri-care, dressing, mouth care, assisting to toilet, incontinence care, repositioning in bed, and assisting with meals
  • Tidy the immediate area and change bed linens for the patient
  • Spend several hours a day in your car driving between patients
  • Maintain an adequate level of supplies in your vehicle to provide patient care
  • Pick up new supplies at the branch office in your area
  • Work closely with each RN case manager to communicate patient status changes, as well as patient or family concerns
  • Train the caregivers to provide personal care to their loved one, while remaining within your scope of practice
  • Coordinate visit times with the patient or caregiver for all assigned patients
