About The Position
Hosparus Health is seeking a compassionate Certified Nursing Assistant (CNA) to join their team, committed to creating more moments for patients facing advanced illness or end-of-life care. The Certified Nursing Assistant (CNA) is responsible for the delivery of personal care and minimal homemaker services (i.e., services promoting a safe and clean patient environment) to hospice patients in their home, assisted living facility, or long-term care facility. These services are provided based on Hosparus Health policies and according to the CNA plan of care (POC). The CNA also participates in the development of policies and procedures that guide the provision of hospice care.
Requirements
- CNA certification required in the state hired to work, or an active LPN or RN license
- Valid Driver’s License
- Auto Insurance coverage
Nice To Haves
- High school diploma preferred
- One year home health aide or nursing assistant experience preferred
- Use of technology-based tools, including electronic medical record, strongly preferred
Responsibilities
- Delivery of personal care and minimal homemaker services (i.e. services promoting a safe and clean patient environment) to hospice patients in the home, assisted living facility, or long-term care facility
- Provide services based on Hosparus Health policies and according to the CNA plan of care (POC)
- Participate in the development of policies and procedures that guide the provision of hospice care
