About The Position

We are looking for kind and compassionate Home Health Aides (HHA) to join our team of professional healthcare staff. You will assist patients with daily tasks while making a meaningful difference in their lives. This position involves providing care across diverse healthcare settings, including home care, long term care, acute, sub-acute, skilled nursing facilities, outpatient facilities, and assisted living facilities. Requirements

  • Dementia experience required in order to understand patient needs effectively.
  • Familiarity with HIPAA regulations to ensure patient privacy is maintained.
  • Compassionate demeanor with excellent interpersonal skills for effective patient interaction.
  • Ability to work well in a team-oriented environment while also being self-motivated.
  • Ensure compliance with HIPAA regulations to protect patient confidentiality.
  • CNA/HHA (Required)
  • Ability to Commute: Cape Cod, MA

Nice To Haves

  • Proficiency in using electronic health record systems is preferred.
  • Experience in home care, nursing homes or hospice care settings is a plus.
  • Certification in Basic Life Support (BLS) is highly desirable.
  • Document patient care activities accurately using electronic health record systems like Epic or eClinicalWorks.

Responsibilities

  • Assist patients with daily activities such as bathing, dressing, grooming, and eating.
  • Meal Prep: Planning, cooking, and feeding according to specific dietary restrictions.
  • Assisting with transportation for errands, doctor’s visit, outings etc.
  • Provide emotional support and companionship to patients.
  • Help maintain a clean and safe environment for patients.
  • Monitor and record vital signs, including temperature, pulse, and blood pressure.
  • Assist with mobility and transportation of patients within the facility.
  • Communicate effectively with nursing staff regarding patient needs and changes in condition.
