About The Position
We are seeking a compassionate and reliable Certified Nursing Assistant (CNA) to support patients served by Select Hospice. In this role, you will provide hands-on personal care in the home, focusing on comfort, dignity, and quality of life while assisting patients and families during end-of-life care under the direction of the RN.
Requirements
- Current CNA or HHA license in the state of Indiana (required)
- Minimum 1 year of direct patient care experience (required)
- Listed on the Nurse Aide Registry with no findings of abuse or neglect
Nice To Haves
- High school diploma or equivalent (preferred)
Responsibilities
- Provide personal care including bathing, dressing, oral care, feeding, and elimination
- Assist with ambulation, positioning, transfers, and ADLs
- Measure and document vital signs and observations as directed
- Travel to and care for multiple hospice patients per day in their residences, following individualized plans of care
- Observe and report changes in patient condition or home safety concerns
- Follow RN care plans and agency policies
- Maintain patient confidentiality and HIPAA compliance
- Complete required trainings, meetings, and in-services
- Support patient comfort through socialization and emotional presence
