About The Position

Integritus Healthcare, specifically Linda Manor Extended Care, is seeking a Certified Nursing Assistant (CNA) to provide high-quality nursing care to residents. Requirements

  • Certified Nursing Assistant (CNA) certification.
  • Knowledgeable of the individualized care plan for residents.
  • Demonstrate respect, attention, and awareness toward the diversity among residents, families, and caregivers.
  • Ability to document ADLs in an electronic health record.
  • Ability to observe and report symptoms and changes in condition.
  • Ability to use appropriate assistive devices and body mechanics for ambulation and transfers.
  • Ability to operate equipment safely and report defects.

Nice To Haves

  • Experience in short-term rehabilitation, long-term care, or specialized dementia care.
  • Participation in QAPI process or as an area of development.

Responsibilities

  • Provide quality nursing care to residents in accordance with their assessment and care plan.
  • Assist residents with daily requirements and tasks, including personal hygiene, dressing, mobility, and food intake.
  • Maintain resident rights and confidentiality.
  • Document all Activities of Daily Living (ADLs) in the electronic health record (EHR).
  • Observe and report resident symptoms, reactions, and changes in condition promptly.
  • Routinely turn and position residents as required by the care plan.
  • Promptly report any incidents or evidence of resident abuse or violations of resident rights.
  • Honor resident's right to refuse care and report such refusal to the nurse supervisor.
  • Assist in maintaining a safe, neat, and clean environment.
  • Answer resident call bells promptly and courteously.
  • Ensure residents who are unable to call for help are checked frequently.
  • Take and document vital signs, food and fluid intake/output, and heights and weights.
  • Ambulates and transfers residents using appropriate assistive devices and body mechanics.
  • Operate equipment in a safe manner and report any defective equipment.
  • Communicate and interact effectively with residents, visitors, families, peers, and supervisors.
  • Assist and escort residents to appointments and activities.
