About The Position

San Francisco Post-Acute is seeking compassionate, dependable, and team-oriented Certified Nursing Assistants to provide resident-centered care in a fast-paced post-acute and skilled nursing environment. The role involves working collaboratively with the healthcare team to support residents’ comfort, dignity, and independence. Part-time shifts are available: AM shifts on weekdays, PM shifts on weekends, and NOC shifts on weekdays and weekends. Full-time employee benefits are also listed.

Requirements

• Current and active Certified Nursing Assistant (CNA) certification.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team-oriented healthcare environment.
• Compassionate, dependable, and committed to resident-centered care.

Nice To Haves

• Previous skilled nursing, rehabilitation, or post-acute experience is preferred but not required.

Responsibilities

• Monitor resident conditions and promptly report changes to the nursing team.
• Respond quickly and professionally to resident call lights and requests.
• Assist residents with activities of daily living while promoting comfort, dignity, and independence.
• Support admissions, transfers, and discharge procedures.
• Assist with meal service and tray distribution as needed.
• Maintain accurate documentation and follow facility policies and procedures.
• Collaborate with nurses, therapists, physicians, and fellow team members to provide resident care.
• Help maintain a safe, clean, and welcoming environment for residents.
