About The Position
Dependable Hospice is seeking a compassionate and dedicated Certified Nursing Assistant to join its Phoenix hospice team. This full-time, field-based role provides one-on-one hospice care to patients in their homes, with a focus on comfort, dignity, safety, and exceptional end-of-life care. The CNA works independently and collaborates with the interdisciplinary hospice team, while providing support to patients and their families.
Requirements
- Current Arizona CNA certification.
- Current CPR certification.
- Valid driver's license and reliable transportation.
- Compassionate and dependable disposition.
- Comfort working independently.
- Excellent communication and organizational skills.
Nice To Haves
- Hospice or home health experience.
Responsibilities
- Provide compassionate personal care to hospice patients in their homes.
- Assist patients with bathing, dressing, grooming, and activities of daily living.
- Promote patient comfort, dignity, and safety.
- Observe and report changes in patient conditions to the hospice nurse.
- Provide emotional support to patients and families.
- Collaborate with the interdisciplinary hospice team.
- Complete accurate and timely visit documentation.
