About The Position

The Certified Nursing Aide provides individualized care to residents at Montclare Supportive Living in Chicago, Illinois. The aide follows each resident’s care plan, supports residents with activities of daily living while promoting independence and dignity, and reports changes in residents’ health status to the clinical team. The role also involves building compassionate, positive connections with residents and family members, participating in learning and development activities, and protecting the confidentiality and privacy of resident information.

Requirements

• High school graduate or GED equivalent.
• Current certification from an accredited Certified Nursing Assistant (CNA) program.
• Exceptional communication and interpersonal skills.
• Ability to serve with love, compassion, and dignity.

Responsibilities

• Deliver exceptional, individualized care as determined by each resident’s care plan.
• Assist residents with activities of daily living (ADLs), promoting independence and maintaining dignity.
• Report changes in residents’ health status to the clinical team.
• Connect compassionately with residents and family members to create a positive experience.
• Attend and participate in educational seminars, staff meetings, and other learning experiences.
• Comply with rules and regulations regarding confidentiality and privacy of resident information.
• Perform other duties as assigned.
