About The Position

VitalCaring is hiring a Home Health Aide to provide compassionate, hands-on support to patients in their homes. The role supports patient comfort, safety, dignity, and independence through personal care and assistance with daily activities. The aide works independently during home visits and collaborates with nurses, therapists, and other care-team members to coordinate care and communicate changes in patient condition.

Requirements

Meet state and federal requirements for Home Health Aide certification or training.
Be at least 18 years of age.
Be able to read, follow care plans, and document services provided.
Hold current CPR certification.
Have a valid driver’s license, reliable transportation, and auto insurance.
Be able to work independently with strong time-management skills.
Demonstrate empathy, patience, and professionalism.

Responsibilities

Assist patients with daily living activities, including bathing, dressing, grooming, and toileting.
Support mobility, transfers, and basic exercises in alignment with the care plan.
Help maintain a clean, safe, and comfortable home environment.
Prepare meals and assist with feeding as needed.
Take and record vital signs when required and trained to do so.
Assist with medication self-administration as directed.
Observe and report changes in patient condition to the clinical team.
Document care provided accurately and promptly in the EMR system.
Participate in team meetings, trainings, and care coordination activities.
Provide compassionate support to patients and families during each visit.
