About The Position

Southwoods Health is hiring a Certified Nurse Practitioner or Physician Assistant for a primary care role in the Boardman/Poland area. The position is full-time with consistent daytime hours and no late nights or weekends. The clinician will provide patient-focused care under general direction as part of a collaborative medical team, helping deliver comprehensive care in a welcoming environment.

Requirements

• Active, valid Ohio license as a Certified Nurse Practitioner or Physician Assistant.
• Active DEA license.
• Current Basic Life Support (BLS) certification through the American Heart Association (AHA).
• Exceptional clinical judgment.
• Strong communication skills.
• Dedicated, team-oriented approach to patient panel management.

Responsibilities

• Diagnose and treat critical, prolonged, and long-term primary care health issues under general direction.
• Identify, evaluate, and address disease prevention while providing high-quality care.
• Communicate and collaborate with medical staff regarding patient consultations, referrals, and specialized care plans.
• Educate patients and their families about preventive care, medical issues, and the proper use of prescribed medications and treatments.
• Maintain accurate, legible medical records documenting evaluations, diagnoses, procedures, treatments, prognoses, and consultations in accordance with state regulatory standards.
• Manage a patient panel as part of a team-oriented clinical practice.
