About The Position

Under the supervision of the RN Care Coordinator Manager, Primary Care, the Certified Medical Assistant performs tasks and has patient contact to assist in coordinating primary care services. This includes obtaining and running reports to help review and resolve care gaps, improve Health Maintenance and HEDIS scores, and coordinate transitions of care from inpatient, Emergency care, and other service settings.

Requirements

Two years of medical assistant experience required.
High school diploma or GED required.
Certified Medical Assistant (CMA) certification required.
Basic Life Support (BLS) certification required at time of hire.

Nice To Haves

One year of experience in GI or a procedural specialty preferred.

Responsibilities

Review monthly Care Gap data from the Managed Care Department for Family Medicine and General Medicine clinics.
Obtain and review daily ED visit reports to help identify high ED utilizers.
Contact insurance Case Managers under the direction of the RN Care Coordinator Manager and implement patient follow-up plans, including phone calls and video discussions to help prevent or reduce ED visits.
Review each clinic's monthly Healthy Planet Performance, identify measures requiring improvement, and communicate findings to the RN Care Coordinator Manager.
Ensure compliance with Health Maintenance and Centers for Medicare & Medicaid Services (CMS) guidelines.
Assist with distributing reports to individual clinics.
Answer questions, assist with problems, and help ensure goals, standards, and deadlines are met.
Assist with maintaining the database and relationships with community resource providers.
Maintain a positive, customer-service-oriented disposition toward clients and coworkers and provide excellent service to internal and external customers.
Prepare and maintain medical-record clerical work according to established guidelines.
Perform additional duties as assigned.
