This role involves preparing patients for provider visits by taking vital signs and assisting with diagnostic procedures. A key responsibility is the organization and coordination of Suboxone and Vivitrol schedules, managing patient distribution across sites and provider time blocks, and maintaining a provider registry for patients receiving MAT. The position also supports patients through various treatment phases, tracks and follows up on diagnostic services and results, and communicates with providers, patients, and families. The role requires maintaining a treatment database with health information and attending mandatory meetings and trainings. The individual will work independently on projects and must demonstrate commitment to the health center's mission and values.
