About The Position
Vitality Hospice & Palliative Care is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join their growing interdisciplinary team. In this field-based role, the CNA will provide hands-on care and emotional support to patients in their homes, helping preserve comfort, dignity, and quality of life during their end-of-life journey. This position is ideal for a CNA who values meaningful work, autonomy, and a supportive team culture.
Requirements
- Active Certified Nursing Assistant (CNA) certification (required)
- Valid driver’s license, reliable vehicle, and current auto insurance
- Ability to work independently in the field
- Strong communication, time-management, and organizational skills
- Compassionate, dependable, and patient-focused
Nice To Haves
- Experience with hospice, palliative care, or chronic illness preferred
Responsibilities
- Assist patients with mobility, transfers, and ambulation
- Provide personal care including bathing, grooming, dressing, and feeding
- Offer companionship and emotional support to patients and families
- Perform light housekeeping tasks related to patient care
- Accurately document care provided in a timely manner
- Follow individualized care plans and hospice standards
- Support a culture of compassion, respect, and patient-centered care
