About The Position
Provides personal care and rehabilitative services and assists clients with activities of daily living under the direction of the Director of Clinical Services or supervising Registered Nurse. Participates in developing and implementing a realistic, goal-oriented plan of care. Supports clients with nutrition, medication assistance, personal care, mobility, appointments, social activities, and household duties.

Responsibilities
- Provide personal care and rehabilitative services and assist with activities of daily living as directed by the Director of Clinical Services or supervising Registered Nurse.
- Participate in developing and implementing a realistic, goal-oriented plan of care.
- Prepare grocery lists and meals; assist with feeding and fluid intake; and record nutritional intake.
- Assist clients in administering medication.
- Assist with grooming, bathing, toileting, dressing wounds, and taking and recording simple vital signs.
- Help clients exercise, move around the house, and transfer when required.
- Accompany clients to medical appointments.
- Assist clients with socialization and recreation activities.
- Help with personal housekeeping duties.
