The Certified Diabetes Educator (CDE) is responsible for providing diabetes self-management training and education to patients and their significant others. This role involves implementing and documenting teaching plans, educational interventions, and follow-up plans based on individualized assessments. The CDE will collaborate with patients and families to establish mutual learning and behavior change goals, document outcome evaluations, and revise interventions as needed. Education provided will be based on the current program curriculum and physician input. The CDE will also communicate education and follow-up recommendations to physicians and other members of the multidisciplinary team. Additionally, this role involves billing third-party payers for diabetes outpatient self-management and training, utilizing appropriate coding guidelines. The CDE will conduct and document individualized assessments of medical, psychosocial, and educational history, and educate patients and families on general diabetes education, glucose meter instruction, insulin start, insulin pump start/follow-up, and CGMS start as needed. As a registered nurse, the CDE provides support for quality clinical care, performs various tests/procedures, administers medications, provides patient education and discharge instructions, and acts as a patient advocate. The role also involves accurate documentation, maintaining quality control logs, using triage protocols, serving as a clinical resource to other staff, performing mini-registrations and scheduling, and acting as a liaison between physicians and other medical professionals. The CDE will serve as a resource and mentor in diabetes management for the healthcare team, other departments, and the community, participating in educational programs and staying current with developments in diabetes management and education. The position requires maintaining knowledge of current standards of care, participating in self-assessment for core competency, maintaining professional memberships and certifications, and collaborating with physicians on research findings and protocol implementation. The CDE must maintain CPR certification, participate in staff meetings, adhere to HIPAA policies for medical record release, accurately measure and record vital signs, document patient communications, and promote a safe environment. Other duties as requested will also be performed.
