Requirements

  • Bachelor's Degree in Nursing for Licensed Registered Nurse or Master's of Social Work with an LCSW.
  • 24 months experience in home or community-based services, case management, or a related healthcare environment.
  • Licensed Registered Nurse or Licensed Clinical Social Worker (LCSW).
  • Certified Case Manager (CCM) or ability to obtain one of the aforementioned licenses within 6 months of hire date.
  • 12 months paid professional experience performing duties which encompass the core functions of case management: assessment, planning, implementation, monitoring and reassessment.
  • One year paid experience as an ADvantage case manager.
  • One year paid experience as a manager of personnel.
  • Proven problem-solving skills.
  • Excellent organizational, problem-solving, and critical thinking skills.
  • Ability to work independently while exercising discretion, confidentiality, and professional judgment.
  • Excellent Communication and Collaborative Leadership Skills.

Nice To Haves

  • Master's Degree in Nursing, Social Work, Healthcare Administration, Public Health, or related field.
  • Current professional licensure (RN, LCSW, LPC, or other applicable healthcare credential).

Responsibilities
