Requirements
- Appropriate Arizona Teaching Certificate
- Arizona IVP Fingerprint Clearance Card
- Proof of immunity to rubeola (measles) and rubella (German measles), or proof of MMR immunization.

- Certain endorsement/approved areas(s) may be required for appropriately certified purposes.
