About The Position

Under direction from the Central Intake and Outreach Coordinator, this role is responsible for receiving and processing referrals to home visiting and early intervention programs in 17 counties in Northwest Ohio as part of the statewide central intake and referral system. The position involves live data entry into multiple data systems, tracking performance indicators, and generating follow-up communication to parents/caregivers and referral sources. Additionally, the role is responsible for providing information to potential program participants, providers, and referral sources about available programs and services in the region for expectant parents and parents/caregivers of young children.

Requirements

High School Diploma or GED and four years of supervised experience in Customer Service, Social Service or Human and Family Development fields; OR
Associate's degree from an accredited college or university in any field AND at least two years of supervised, paid experience in case management.

Responsibilities

On a daily basis, receive referrals for home visiting and early intervention programs in the Northwest Ohio Region of central intake through telephone, fax, email, and statewide electronic data system(s).
Respond to telephone calls from families, referral sources, and service providers.
Check fax, email, and data system(s) regularly for new referrals.
Enter referral information in data systems.
Assign new referrals to agencies following Bright Beginnings policy and procedures.
Make verbal or written contact with the family within required timelines.
Assist referral sources with questions about referral forms/process.
Follow up to referral sources within required timelines using required forms/process, including information regarding the status of the referral.
Maintain waiting list of families interested in home visiting services for whom no provider is currently available.
Assist service providers in the region with referral issues as needed.
Maintain child and family records in a secure and confidential manner.
Complete risk assessments with sensitivity.
Catalog incoming/outgoing documentation, phone calls, records, and other forms of communication.
Make referrals to other community programs when applicable.
Determine home visiting eligibility as applicable.
Provide current and accurate information about home visiting, early intervention, and community resources to families and professionals who contact central intake.
Identify any potential problem areas and report to Central Intake & Outreach Coordinator.
Complete required and recommended training based on central intake Professional Development Plan and individual performance appraisals.
Participate in outreach events in NW Region when requested by supervisor.
Participate in monthly supervision and team meetings with central intake and all-staff meetings with Lucas County Family Council.
