About The Position

The CDS Coordinator of Community Integration is responsible for coordinating, supervising and documenting the community activities of the Community Day Services program. This individual is responsible for scheduling community integration activities, coordinating with Transportation Services and supervises and monitors the DSP's. While in the community, the CDS Coordinator assures that personal care, skills training, and program mission are carried out throughout the experience. This individual is responsible for enhancing relations using effective communication skills, encouraging growth and development of clients and staff, facilitating teamwork and developing community relationships.

Requirements

  • High school diploma or equivalent
  • 2 years experience with working with people with developmental disabilities
  • Valid Illinois Driver's License
  • Maintain minimum insurance requirement
  • Pass physical function test
  • Must have or be able to obtain CPR, Heimlich Maneuver, First aid qualifications
  • Data entry skills using programs such as VERTEX, Paychex, and Relias
  • Solid communication and interpersonal skills

Responsibilities

  • Coordinate and communicate the scheduling of community integration activities with CDS management, coordinators and direct support staff, and provide transportation as needed.
  • Coordinate and communicate with transportation services, assuring transportation policies are followed
  • Supervise direct support persons as they support participants in various community scheduled activities and events
  • Address personal care and wellness needs of program participants
  • Coordinate and communicate with staff to assure that all areas are covered throughout the community activity, preparation and follow up
  • Assure that staff support and encourage independence and self-reliance, allowing program participants to achieve their greatest potential by giving them opportunities to make informed decisions and to make choices about their daily routines
  • Assist CDS team in developing community interactions that address the needs and desires of program participants
  • Coordinate and communicate with QIDPs, residential providers, participant's guardian/family/ caregiver and other agency staff to assure quality community opportunities and provision of services
  • Assure attendance, client timecards and service hours are completed and accurate, using current technology associated with the task
  • Complete incident /injury reports, critical incident and seizure reports in a timely manner
  • Ensure safety of program participants
  • Be alert to individual medical needs, provide first aid and contact supervisor, or executive staff member and or agency health professional when necessary
  • Meet regularly with CDS Team to address issues related to individual participant's outcomes, objectives and choice as well as overall functioning of the program
  • Coordinate with the Managers of Operations; addressing expectations of DSP's and give input in annual evaluations
  • Assist CDS Manager of Operations with orientation and training of new DSP's, and with the orientation of new clients.
  • Follow DSP training and job description
  • Provide coverage at CDS Sites or CILA's as needed
  • Responsible for attending trainings to meet annual requirements: First Aid, CPR, CPI, Safety
  • Respect persons served and promote a positive image of person with disabilities
  • Perform other duties as assigned
