About The Position

The Solo With Supports (SWS) Program helps youth build the life skills, confidence, and independence they need as they transition into adulthood. Youth may live in a mentor support home or in CHIMO's Transition Home while receiving individualized support. As a Youth Coordinator, you'll support a caseload of youth by providing guidance, case management, and hands-on support to help them reach their goals.

Requirements

  • A diploma or degree in Child and Youth Care, Disability Services, Social Work, Psychology, Community Rehabilitation, or a related field.
  • At least two (2) years of experience working with children, youth, or young adults.
  • Eligible to work in Canada.
  • A current Police Information Check and Intervention Record Check (issued within the past six months).
  • A valid Alberta Driver's License.
  • At least three (3) years of driving experience with a clean three-year Driver's Abstract and Driving Record, with no at-fault claims.
  • Access to a reliable vehicle, as regular driving in the community is required.

Responsibilities

  • Help youth build the skills and confidence they need to thrive as adults.
  • Develop, monitor, and update Safety Plans and Service Plans.
  • Work closely with youth, families, support networks, and community partners to provide coordinated care.
  • Connect youth with community resources and advocate for their needs.
  • Keep accurate case notes, reports, and documentation.
