Perform initial intake, screening, and problem identification of inquiries from those seeking information/assistance or assessments and referrals for home and community-based services. This role involves receiving inquiries, conducting problem identification, providing information and assistance about resources, and making referrals. A significant portion of the role involves assisting with applications for special programs and maintaining updated community resource information. The position also includes participating in public education opportunities and representing the agency. Other duties include maintaining consumer contact records, attending trainings, and serving as an advocate for older consumers.
