Performs comprehensive assessments for persons seeking long term care services and needing public funding or home and community-based services. The position involves making face-to-face visits with consumers to complete thorough assessments, reviewing medical and healthcare needs to issue the most appropriate and least restrictive Level of Care. It also includes completing required forms and documents for Dom Care Home and Personal Care Home residents applying for state SSI supplement, performing Level II assessments, and maintaining accurate records of consumer assessments. The role requires knowledge of WellSky and PIA software and adherence to Pennsylvania Department of Aging (PDA) guidelines. The caseworker must also be knowledgeable about aging program policies, community services, and provide benefits counseling. The position requires the employee to provide their own transportation for home visits and other job-related appointments.
