About The Position

The Division of Aging and Adult Services (DAAS), within the Department of Health and Human Services (DHHS), is looking for an APS Investigator (Caseworker II) for the Northern Utah Counties. The APS Investigator provides adult protection services (APS) by investigating and assessing allegations of abuse, neglect, and exploitation on vulnerable adults, and documenting investigative findings and client progress toward risk resolution. They conduct Quality Assurance Reviews and carry out other tasks as assigned. This position requires in-state travel as part of the normal workload.

Requirements

Requires a Social Service Worker license or a closely related license in the State of Utah
Has at least six months of employment as a case manager or investigator
Knows investigative techniques and how to implement the techniques
Must successfully pass a criminal history check

Responsibilities

Investigating and assessing allegations of abuse, neglect, and exploitation on vulnerable adults
Documenting investigative findings and client progress toward risk resolution
Conducting Quality Assurance Reviews
Carrying out other tasks as assigned
Collect required documentation and fill out application forms for public assistance programs (Medicaid, Medicare, Social Security, SSI, SNAP)
Assist guardians in accessing pensions and other sources of income
Collect and organize client bank statements, bills, and receipts and assist guardian in tracking expenses
Check and log balance statements for client personal needs accounts (PNA) at resident facilities to verify fund availability
Input historical care date, visitation logs, and financial timelines into the agency database to assist the guardian in preparing court paperwork
Assist management in administrative roles
