About The Position

The Caseworker Coach II-Mobile Outreach Worker and Housing Navigation position combines outreach, case management, and housing navigation to support women experiencing homelessness and living with behavioral health conditions. This role provides both community-based outreach and onsite services through an individualized, participant-centered approach. The Caseworker-Coach II assesses participant needs and connects women to recovery-oriented resources, behavioral health services, housing opportunities, and supportive services throughout the Central San Diego region, including services available through Rachel’s Women's Center.

Requirements

  • Associate degree and a minimum of two (2) years of experience in case management, housing navigation, outreach services, or a related field; equivalent experience may be considered.
  • Minimum of two (2) years of experience serving unhoused adults with co-occurring substance use disorders and/or serious mental illness.
  • Demonstrated understanding of behavioral health and recovery-oriented service models.
  • Experience working with women and women's services programs.
  • Experience utilizing trauma-informed care, motivational interviewing, harm reduction principles, and the Stages of Change model.
  • Understanding of and commitment to Housing First principles.
  • Valid California Driver’s License with an acceptable driving record.
  • Passing a background check and TB test.
  • Fluency in English required; bilingual proficiency in Spanish or another threshold language preferred.
  • Proficiency with computer systems and the ability to learn and accurately utilize electronic documentation platforms.
  • Ability to read, understand, and interpret policies, procedures, operating instructions, and safety guidelines.
  • Strong organizational, prioritization, time management, and multitasking skills.
  • Ability to work independently while contributing effectively as part of a multidisciplinary team.

Nice To Haves

  • Bilingual proficiency in Spanish or another threshold language preferred.

Responsibilities

  • Conduct mobile and street outreach to build rapport and establish trust with women experiencing homelessness.
  • Identify and help remove barriers to housing, healthcare, behavioral health services, and other community resources.
  • Engage participants in Diversion conversations and administer the VI-SPDAT, as appropriate, to support housing placement efforts.
  • Provide transportation to Rachel’s Women's Center or other community services as needed.
  • Reconnect participants with behavioral health providers and advocate for their needs with external agencies and internal Catholic Charities programs.
  • Accompany participants to appointments and advocate on their behalf with service providers, landlords, and housing agencies.
  • Facilitate group presentations, workshops, and one-on-one coaching sessions.
  • Accurately document services provided using program data systems, including MHOMS and Clarity HMIS.
  • Adhere to the mission, values, policies, and procedures of Catholic Charities, including those related to confidentiality and the appropriate use of electronic devices.
  • Maintain comprehensive and current knowledge of community resources serving individuals experiencing homelessness.
  • Other duties as assigned.
