About The Position

This position is responsible for monitoring an assigned caseload of clients participating in short-term life management. The Case Worker will establish program plans/goals and evaluate client progress by conducting routine meetings with clients and/or staff. Key responsibilities include interviewing applicants, conducting needs assessments, developing intake and case plans, instructing clients in completing necessary paperwork, meeting regularly with clients to discuss and evaluate progress, preparing accurate and up-to-date records, receiving incident reports, and enforcing disciplinary actions for infractions of house guidelines.

Requirements

Two years of college coursework from an accredited college or university in Social Work, Behavioral Science, or a related field, AND Two years experience working in a social or public service environment with experience assisting the public OR Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
Ability to read, write, and communicate the English and Spanish languages.
Valid State Drivers License.
Requires a level 2 background screening.

Responsibilities

Interviews applicants based on established guidelines and requirements.
Conducts needs assessment, obtains pertinent information, and establishes program goals for eligible clients.
Develops intake and case plans for all assigned clients.
Instructs clients in completing necessary paperwork including goals and/or budget forms.
Meets regularly with clients to discuss and evaluate their progress.
Prepares accurate and up-to-date records documenting the same.
Receives incident reports and enforces disciplinary actions for infractions of the house guidelines.
