About The Position

The Clinic Case Worker coordinates services that support patient satisfaction, high-quality care, and cost-effective health outcomes. This position promotes the physical, psychosocial, and vocational well-being of patients by ensuring care is delivered in the most appropriate setting. The Clinic Case Worker proactively monitors patient registries to identify and coordinate healthcare needs and works closely with clinic patient care teams.

Requirements

  • Graduate of an accredited school of nursing
  • Current RN license in Oregon without restriction
  • BLS certification required
  • Excellent verbal and written communication for interacting with patients, families, providers, and coworkers.
  • Ability to identify problems and find effective solutions.
  • Ability to remain calm and focused in urgent or stressful situations.
  • Ability to advocate for patients by working with insurance and other parties to secure necessary care.
  • Skilled in educating patients about their care and treatment options.
  • Strong ability to multitask and manage time effectively in a busy clinic environment.
  • Competent in using electronic health record (EHR) systems for documentation and ordering.
  • Must be able to perform the primary duties and responsibilities of the job with or without reasonable accommodation.

Nice To Haves

  • BSN preferred
  • Experience in case management and patient empanelment, preferred.

Responsibilities

  • Serve as a communication liaison between patients, families, and the care team—relaying messages, gathering clinical and social information, and supporting patient-provider interactions.
  • Proactively monitor patient registries to identify care needs, coordinate preventive and follow-up services, and ensure accurate data entry and tracking for reporting and outcomes.
  • Coordinate care for patients with complex medical and psychosocial needs, including discharge planning, chronic disease management, and connections to community resources.
  • Work collaboratively with primary care providers and interdisciplinary team members to develop and implement individualized care plans that support optimal health outcomes.
  • Identify and help resolve social service issues affecting care delivery in both outpatient and hospital settings, acting as a patient advocate and resource connector.
  • Maintain comprehensive and up-to-date documentation of care coordination activities, including registry updates, case notes, and performance metrics.
  • Demonstrate knowledge of patient-centered care program requirements and assist with required reporting, including data gathering and outcome evaluation.
  • Support chronic disease management efforts for conditions such as diabetes, asthma, CHF, and depression, using a population health model and registry data.
  • Coordinate with pharmacies, insurers, and external healthcare providers to ensure timely and accurate exchange of patient information and continuity of care.
  • Uphold legal, ethical, and professional standards related to patient privacy, consent, benefits, and care advocacy while maintaining a respectful, patient-centered approach.
