About The Position

Serenity Knolls, located in the San Geronimo Valley, is seeking a full-time Case Manager to join their team. Serenity Knolls is a 42-bed residential treatment center for individuals aged 18 and older, including 6 detoxification beds. The facility utilizes a 12-step based, social model approach, integrating traditional social model care with modern cognitive behavioral, psychodynamic, and evidence-based practices.

Requirements

• Bachelor's Degree in Social Work or in a human service-related field.
• Ability to self-motivate and possess strong time-management and organizational skills while managing a high volume of client interactions.

Responsibilities

• Acts as a liaison for patients with the referral network.
• Assists patients, in agreement with their counselor, to seek social, vocational, and rehabilitative services as needed.
• Directs residents in a clear, concise, and informative manner.
• Contributes to the Comprehensive Treatment Plan through participation in treatment planning meetings and processes.
• Completes any necessary client-centered paperwork in a timely manner.
• Demonstrates good moral character and even temperament necessary to gain and maintain credibility and trust of persons being served, peers, and management staff.
