About The Position

Project Restore Brownsville is a community-based violence intervention and prevention initiative designed to support young people at elevated risk of involvement in gun violence. The program combines intensive case management, mentoring, trauma informed group work, and workforce development opportunities to stabilize participants, reduce violent behavior, and support long-term positive outcomes. Project Restore operates as an enhancement of existing BIVO work, strengthening continuity of care, supervision, and participant engagement across systems. Position Overview: What The Case Manager Does: The person filling this position is expected, under close supervision, to provide intensive, participant-centered case management services to Project Restore Brownsville participants. The Case Manager supports participants through individualized service planning, weekly engagement, referrals, and coordination of care to address barriers related to safety, stability, education, employment, and wellness. Practice Universal Precautions/Standard Protocol & Procedures. Comply with all Federal, State, City, and CAMBA security and privacy policies to protect the confidentiality of individual health information. Ensure the safety and security of all participants by following program protocols and procedures, including supervision, accident and incident reporting.

Requirements

  • High School Diploma or General Educational Development (GED) and 2 years of case management experience working with justice-involved or system-impacted youth or young adults, or Bachelor’s degree in social work, human services, psychology, criminal justice, or a related field.
  • Strong understanding of trauma-informed care and community-based service delivery.
  • Experience conducting assessments and developing individualized service plans.
  • Strong organizational, documentation, and time-management skills.
  • Ability to work collaboratively as part of a multidisciplinary team.

Responsibilities

  • Conduct comprehensive intake assessments and develop individualized service plans for assigned participants.
  • Maintain weekly case management contact with each participant, adjusting intensity based on participant need.
  • Coordinate referrals and linkages to education, employment, housing, mental health, substance use, and other supportive services.
  • Support participants in meeting court obligations, probation requirements, and other system-related responsibilities, as applicable.
  • Monitor participant progress toward individualized goals and update service plans accordingly.
  • Collaborate closely with Peer Advocates, Credible Mentors, and clinical staff to ensure coordinated service delivery.
  • Participate in case conferences, supervision sessions, and multidisciplinary team meetings.
  • Support crisis intervention efforts in coordination with senior staff and clinical leadership.
  • Maintain accurate, timely, and thorough documentation of participant contacts, assessments, and outcomes.
  • Track attendance, engagement, and participation in program activities.
  • Assist with participant re-engagement strategies when participation declines.
  • Educate participants on available resources and support self-advocacy skills.
  • Ensure compliance with program requirements, documentation standards, and reporting deadlines.
  • Contribute to program evaluation and continuous quality improvement efforts.
  • Other duties as assigned.
