About The Position

St. Catherine's Center for Children is a non-profit organization dedicated to providing a wide range of human services to vulnerable children, families, and adults in the Greater Capital Region of New York. Our services include residential and therapeutic foster care, Kinship support, homeless and community-based services, and a special education elementary school. Requirements

  • Associate degree in a related field with two years of relevant experience.
  • High School Diploma or GED equivalent required.
  • Previous experience in case planning or case management preferred.
  • Familiarity with public assistance and other local resources.
  • Must possess a current, clean, and valid New York State Driver’s License (actively licensed for at least one year).
  • Must have a reliable vehicle for transporting clients.
  • Proof of 100/300 liability insurance coverage.
  • Must be at least 21 years of age and meet agency insurance criteria for authorized drivers.
  • Proficiency in computer applications, such as Microsoft Office & Google Workspace Suites.
  • Proficiency in secure internet practices.
  • Willingness and ability to work in close proximity with the clients served.
  • Ability to communicate, build respectful relationships with, and relate effectively to a wide range of individuals.
  • Demonstrate empathy, compassion, and a family-focused approach.
  • Excellent organizational, planning, and time-management skills.
  • Ability to maintain composure, patience, and professional boundaries under crisis conditions.

Nice To Haves

  • BSW or Bachelor’s Degree in a Human Services field preferred.
  • Will consider previous professional and/or life experience in lieu of a college degree.
  • Previous experience working in a human services agency is a plus!
  • Knowledge of standards and mandates set by St. Catherine’s Center for Children and external regulatory bodies (OCFS, SED, OMH, OTDA, DOH, etc.).
  • Capable of exercising ethical, independent judgment and discretion while handling sensitive data and client Protected Health Information (PHI) in full compliance with HIPAA requirements and strict confidentiality standards.

Responsibilities

  • Conduct comprehensive assessments to identify family strengths and needs.
  • Develop, monitor, and update individualized service plans in accordance with NYS OTDA requirements.
  • Provide intensive case management to stabilize conditions contributing to homelessness.
  • Offer guidance in self-sufficiency, home maintenance, budgeting, meal preparation, time management, parenting skills, and tenant rights.
  • Assist families in identifying and overcoming barriers to housing retention by connecting them with essential rehabilitative, mental health, educational, employment, and substance abuse services.
  • Coordinate transportation for families to appointments.
  • Serve as the primary advocate and liaison for families navigating public assistance, housing resources, and community service networks.
  • Empower clients to make informed daily choices and build skills for long-term housing independence.
  • Intervene in crisis situations and facilitate conflict resolution.
  • Assist facility staff in service delivery to ensure a safe, secure, clean, and healthy living environment.
  • Maintain timely and complete documentation, including progress notes, incident reports, and assessment forms.
  • Complete required outcome measures for Quality Improvement (QI) and submit administrative reports on time.
  • Establish and maintain respectful, professional working relationships with clients and colleagues.
  • Adhere strictly to personal boundaries, agency policies, and client confidentiality.
  • Participate in regular professional development and attend shelter staff meetings.
  • Clearly communicate program updates or concerns to the Program Director.
  • Perform additional position-related duties and responsibilities as assigned.
