About The Position

The Case Manager role at Strickland is within an intensive residential program that provides 24-hour support services for individuals discharging from CVH. The Case Manager will utilize appropriate therapeutic models to aid clients in understanding and managing their symptoms, maintain family and other collateral contacts to ensure quality, client-centered services, and assess and involve natural supports in treatment planning and discharge processes. This role also involves providing case management, clinical, and crisis intervention supports as indicated, demonstrating competency in age-related or specialty issues and developmental needs for each population served, and using EMR to document all services and contacts with clients, collateral contacts, and appointments.

Requirements

  • Bachelor’s degree in human services required.
  • Valid driver’s license.

Nice To Haves

  • At least two (2) years experience in a mental health setting preferred.
