About The Position

SUS is seeking a bilingual Case Manager for our transitional shelter in Brooklyn. This role involves providing comprehensive case management services to an assigned caseload, including conducting assessments, developing service plans, facilitating counseling sessions, and connecting clients with appropriate resources. The Case Manager will also be responsible for accurate and timely documentation, crisis intervention, and ensuring compliance with agency and funder standards.

Requirements

  • HS Diploma or GED
  • 1-2+ years’ experience with homeless population
  • Ability to work with a culturally diverse population
  • Bilingual candidates are strongly encouraged to apply; Spanish, Creole, or French language skills are a plus
  • Maintain current CPR and First Aid certification

Nice To Haves

  • Previous case management experience in a homeless shelter setting
  • Experience using the AWARDS application
  • Bachelor’s Degree is Human Services field preferred
  • Experience in residential setting, group work, tenant organization, case management, referral, and/or community outreach strongly preferred
  • Bilingual Spanish speaking strongly preferred

Responsibilities

  • Conduct the intake process with new clients within 24 hours of admission, including explaining resident rights and responsibilities and grievance procedures.
  • Conduct and document psychosocial, vocational, educational, and housing assessments.
  • Provide accurate, complete, and timely documentation of supportive services and clients’ progress toward securing permanent, affordable housing.
  • Prepare housing applications and assist clients throughout the housing application and interview process.
  • Monitor and assess changes in clients’ behavior, appearance, and overall well-being.
  • Facilitate individual and group counseling sessions, workshops, psychoeducation, and recreational activities.
  • Conduct room and dorm inspections to assess personal hygiene, cleanliness, and activities of daily living.
  • Provide comprehensive case management services to an assigned caseload.
  • Develop initial service plans and complete service plan reviews as required.
  • Connect clients with appropriate medical, mental health, housing, and community-based supportive services.
  • Provide crisis intervention and respond appropriately to emergency situations.
  • Monitor medication compliance in collaboration with nursing staff.
  • Assist with building evacuations during fire drills and emergencies.
  • Maintain current CPR and First Aid certification.
  • Maintain accurate, complete, and timely client records in accordance with DHS, OTDA, funder, and SUS standards.
