About The Position

The Case Manager is responsible for coordinating safe, timely, and clinically appropriate discharge planning for patients receiving inpatient psychiatric treatment. Working collaboratively with patients, families, treatment teams, community providers, and external agencies, the Case Manager facilitates continuity of care by identifying discharge needs and barriers, coordinating referrals and aftercare services, arranging transportation, and ensuring all discharge planning activities are documented accurately and completed within established timeframes. This role requires proactive case coordination from admission through discharge to support successful patient transitions and positive treatment outcomes.

Requirements

  • Associate Degree required.
  • One to two years of case management, discharge planning, behavioral health, or healthcare experience preferred.
  • Must demonstrate effective care of coordination, referral management, documentation, and collaboration with multidisciplinary teams and community providers.
  • Requires organizational, verbal, interpersonal and customer service skills, grammar and spelling, the ability to read/comprehend written instructions and follow verbal instructions.
  • Ability to establish and maintain effective working relationships with employees, managers, referral sources, patients/families, community resources, etc.
  • Ability to exercise professional independent judgment in planning own work.
  • Must be flexible with work hours to accommodate work volumes.
  • Vision and hearing sufficient to perform the essential functions of the job.

Nice To Haves

  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field required.
  • Knowledge of utilization management and managed care environment is helpful.
