Case Managers (CM) render services to individuals 18 and over focusing on recovery and resiliency principles as they empower individuals experiencing substance use disorders to integrate, function, and succeed in their natural environments. The Case Manager will assess, plan, implement, coordinate, monitor, and evaluate the options and services necessary to meet the needs of clients. The CM also provides symptom monitoring, crisis management, advocacy, and connections to natural and community supports and resources. Case Managers are expected to have the ability to establish working relationships with diverse individuals in a variety of settings and situations and must demonstrate an ability to work collaboratively with others on behalf of the client both within the agency, in the community, and/or family setting. The position requires experience working with adults with mild to moderate mental illness and substance abuse disorders.
