We are seeking a compassionate and organized Clinical Case Manager to join our outpatient ambulatory clinic team. This role is ideal for a licensed RN who is passionate about coordinating care, advocating for patients, and helping individuals successfully transition from hospitalization to outpatient services. The Clinical Case Manager utilizes clinical expertise, critical thinking, and care coordination skills to support safe, effective, and patient-centered care. Working collaboratively with providers, healthcare teams, payers, patients, and families, this case manager coordinates services and assists with outpatient discharge planning to promote positive patient outcomes. This opportunity is for full-time (5 shifts per week) and 8-hour day shifts.

Requirements

  • Bachelor’s Degree in Nursing (BSN)
  • Current California Registered Nurse License
  • Minimum of three (3) years current clinical experience in the area of Case Management.
  • BLS Certification, through the American Heart Association
  • Experience with cancer treatment protocols.
  • Exceptional communication and interpersonal skills, with a focus on patient advocacy and satisfaction.

Nice To Haves

  • Experience in the outpatient setting is highly preferred.
  • OCN, CCM preferred

Responsibilities

  • Collaborate with physicians, clinical staff, payers, and community resources to facilitate continuity of care.
  • Serve as a patient advocate, ensuring access to appropriate services and resources.
  • Process and coordinate referrals for specialty care and support services.
  • Ensure patients understand discharge instructions and medicine changes.
  • Arrange and manage services including: - Durable Medical Equipment (DME), Home Health Services, Wound Care Services, Hospice Care, Respiratory
  • Assist with outpatient discharge planning and transitions of care.
  • Knowledge of utilization review is needed to monitor insurance coverage limitations.
  • Assisting providers with documentation needed to support medical necessity.
  • Maintain accurate and timely documentation in the EPIC electronic medical record system.
