About The Position

CSPNJ, a mental health nonprofit agency, is looking for a Veterans Case Manager. Under the direction of the Director of Veterans, the Case Manager delivers in-person supportive services designed to help veterans maintain independence and achieve their full potential. The Case Manager supports veterans in maintaining housing stability across Bergen, Hudson, and Passaic counties.

Requirements

  • Bachelor's degree in mental health/human service discipline or High School Diploma / GED with equivalent work experience.
  • The ability to write clear, concise, and accurate billable progress notes.
  • Exceptional time management skills to be able to support the needs of multiple veterans.
  • Knowledge of poverty and homelessness causes, trends, issues and solutions.
  • Strong expertise in strengths-based, solution-focused practice.
  • Strong verbal and written communication skills.
  • Knowledge and experience working with diverse cultures and ethnicities.
  • Must be proficient in MS Office products, as well as general computer literacy.
  • Proven ability to engage and support individuals with mental health diagnoses using empathy, professionalism, and trauma-informed practices.
  • MUST have valid NJ Driver’s License with acceptable driving record.

Nice To Haves

  • Certified Peer Specialist (CRSP) plus 1 year of related work/life experience preferred.
  • Bilingual skills are a plus!
  • Lived Experience: CSPNJ prioritizes hiring people who use their own life experience dealing with mental health/substance use issues to inspire and support others.

Responsibilities

  • Record observations, veteran information, and services provided in daily shift notes, weekly progress notes, and other required documentation as needed.
  • Follows team protocol for documenting and recording eligibility assessments (BVH Prioritization Tool, housing barriers assessment tool, and housing stabilization plan developed by DCA).
  • Assess veterans’ daily living skills and provide individualized instruction in areas such as budgeting, cooking, laundry, cleaning, shopping, personal hygiene, and self-advocacy as needed.
  • Conduct crisis intervention as necessary and connect veterans with appropriate emergency services.
  • Participate in community outreach and education efforts to raise awareness of homeless veterans and services.
