About The Position

Axiom Care is a fast-paced, rapidly growing Substance Abuse treatment, Behavior Health and Mental health sober living housing provider. We have several different locations and several programs for individuals seeking to change their life for the better. These different programs include group counseling, recovery education, employment services, art therapy and high needs case management in a sober living environment. These programs serve a variety of specialty populations including SMI, GMH, justice involved, Navajo and other Indigenous individuals, military veterans aiming for recovery, reintegration into the community, and independent living. Specialty Case Managers focus on the various social determinants of health and structural obstacles that are necessary to address for the clients/residents to have the highest chance of successful recovery and community reintegration. Additionally, they assist in on-site activities and functions of the program, as assigned by the Clinical Director, Facilities Manager and/or other administrative staff. Case manager is an integral role in Axiom Care team. Founded in 2012, Axiom Care is a Phoenix-based behavioral health organization that aims to ensure every Arizonan has access to compassionate, effective substance use treatment. With facilities in Maricopa and Pinal Counties, we offer comprehensive services including 24/7 observation, inpatient detoxification, residential treatment, outpatient treatment, and recovery housing with a focus on underserved populations. Beyond our full-continuum community programs, we proudly operate specialty programs for the justice-involved (in partnership with ADCRR), for members of the Navajo Nation (in partnership with the Navajo Department of Health), and for Veterans (McCain Veteran Center). Requirements

  • Must be proficient in computer applications and electronic medical record systems.
  • Proficient with Microsoft Office Suite or related software.
  • Some assignments require an Arizona Fingerprint Clearance Card
  • Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR).
  • Ability to operate 15-passenger van
  • Active CPR and First Aid certification
  • Negative TB Test result within past 12 months (residential locations).
  • Ability to write professional correspondence and reports free from grammatical and spelling mistakes.
  • Ability to read, analyze, and interpret emails, letters, policies, procedures, etc.
  • Ability to communicate effectively with clients, client support and professionals.
  • Self-motivated and self-directed
  • High level of patience
  • Ability to make sound decisions in emergency situations.
  • Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the public
  • Ability to define problems, collect data, establish facts, and draw valid conclusions
  • Strict adherence to ethical treatment standards
  • Strong time management skills
  • Personal or professional experience in recovery or substance use treatment.
  • Familiarity with 12-Step programs, VA systems, or trauma-informed care.
  • Minimum one year of case management knowledge and experience with the AHCCCS population chronically underhoused and the substance use treatment), Justice involved persons, veterans/military population, and Indigenous population.

  • One year of experience with the Crisis/SMI population strongly preferred.

Responsibilities

  • Maintain strict adherence to state and local ethical standards and legal requirements at all times
  • Facilitate and document group sessions, focusing on recovery, life skills, and other relevant topics.
  • Carry a caseload of clients, guiding them through all program phases
  • Develop and implement client-driven treatment plans tailored to individual needs
  • Secure client information per HIPAA standards
  • Complete high-quality documentation of any service provided, follow up needed, or other clinically pertinent information within 24 hours in the electronic medical record
  • Take initiative to be aware and stay updated about community resources
  • Assess client progress, needs and barriers in each interaction and document
  • Identify and report ongoing maintenance and safety issues at facility
  • Intervene with de-escalation, empathy and, as needed, emergency response protocol to crisis and emergency situations
  • Attend frequent site staff supervision and ongoing staff trainings.
  • Transport clients to appointments as needed.
  • Utilize coaching and motivational techniques to help clients achieve their treatment goals.
  • Assist in holding clients accountable for attendance and compliance with program expectations
  • Perform other related tasks and projects as assigned by management
