As a Case Manager you will work independently and as part of a team that co-ordinates a support network and links clients living with mental illnesses, addictions and physical health issues who live in a congregate living setting to various community services. The position attempts to empower the client, thereby increasing their independence, dignity and self-respect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs. The York Region/South Simcoe Jane Street High Support Program (Congregate Living program) is staffed 16 hours of the day and provides case management, residential and life enrichment supports to adults, aged 16+. The Case Manager works as part of a team, along side Residential Support Workers to assist individuals in meeting their basic needs (e.g., food, shelter, finances) and encourage the development of skills needed to live independently and improve quality of life.
