About The Position

Community Solutions, Inc. (CSI) is seeking a dedicated and passionate Case Manager to join our team at our new Multi-Dimensional Anti-Recidivism Partnership (MAP) located in West Valley, CA. Adult MAP is an innovative program dedicated to engaging the community and providing rehabilitative programming for justice-involved individuals. We focus on promoting long-term, self-sustaining skills through vocational training and personal development services. This unique model hosts a variety of sub-contractors at one site to reduce barriers for clients. As a Case Manager at our MAP programs, you will be working closely with the Program Director, providing re-entry services to a criminal justice impacted population. You will be responsible for computerized facility information and will complete reports as needed/directed.

Requirements

  • Bachelor’s degree and one year experience in case management or similar field.
  • Education may be substituted with experience on a year for year basis.
  • Strong decision-making, organizational, and interpersonal skills.
  • Experience using Microsoft Office and a commitment to working as part of a team.

Nice To Haves

  • Previous assessment and facilitation experience preferred.

Responsibilities

  • Perform client needs assessments within seven (7) days of intake and enter data into CSI data systems on the day the data is obtained.
  • Enter into funder database within five (5) days of client advancement into Services.
  • Actively participate in Client Progress Review (CPR) meetings by presenting new clients, assessments and Individual Service Plan updates on clients as well as discharge planning for clients nearing discharge.
  • Provide and elicit peer support for client issues.
  • Develop and document program plan goals with clients within thirty (30) days of activation and review with client on a bi-weekly basis.
  • Enter goals and relevant client data into Individual service Case Plan and ensure ARMS database reflects case planning efforts and real time progress.
  • Monitor program stipulations, job readiness preparation, and community resource referrals.
  • Entering all new referrals to interventions they facilitate into the CSI data systems on the day of the referral.
  • Prepare, plan and facilitate individual and group sessions with integrity and fidelity to curriculum/manual.
  • Schedule and conduct make-up sessions as needed.
  • Documentation of group and individual sessions are to be entered into the agency data systems on the same day the information is received as well as funder database.
  • Maintain a minimum of 75% completion rate for each intervention facilitated.
  • Maintain a working knowledge of and abide by the Agency’s programs, policies, procedures and code of ethics.
  • Perform other tasks as assigned by supervisor.
