About The Position

Odyssey House Louisiana, Inc. is seeking a full-time Case Manager to join their mission of providing a comprehensive continuum of care for people in Louisiana, encompassing primary care, behavioral health, and substance use disorder treatment. The Case Manager will be responsible for determining eligibility for guests to participate in the Agency’s Low Barrier Shelter (LBS) and will utilize Motivational Interviewing to assess guest needs and disabilities to create individualized housing plans.

Requirements

  • Valid driver's license
  • Excellent communication and organizational skills
  • At least two years of case management experience
  • Database management experience
  • Ability to work with homeless guests of diverse backgrounds and disabilities including physical, mental, substance abuse, immunodeficiencies, etc.

Responsibilities

  • Provide case management to approximately thirty or more guest(s) as assigned, including screening, intake, assessment, individualized housing plan development, monitoring, linkage to appropriate community resources, follow up, and transition to housing.
  • Assist guest(s) with obtaining vital records, ID cards, Driver’s Licenses, and other necessary documentation.
  • Assist guest(s) with completing lease agreements and obtaining supportive documents required by leasing agents/property.
  • Assess guest(s) Medicaid status and follow agency policies to obtain and maintain Medicaid status.
  • Enter guest information into agency database(s) in a timely manner and update as needed.
  • Prepare and submit reports to Project Manager in a timely manner.
  • Maintain up-to-date information regarding tenant occupancy, move-outs, and vacancies.
  • Utilize Motivational Interviewing (MI) to elicit behavior change by guiding participants to explore and resolve ambivalence.
  • Ensure guest(s) are referred to and/or aware of available vocational, educational, housing, and social services in the community.
  • Assist guest(s) in developing life skills necessary to navigate the environment, access community resources, pursue educational and occupational opportunities, obtain housing, manage finances, and access public assistance.
  • Attend all required meetings and trainings.
  • Keep written progress notes of work done on the case.
  • Make necessary referrals to fill potential gaps in assistance.
  • Verify the results of assistance provided to close the case, as per the approval of the Program Manager.
  • Provide crisis management when indicated by guest(s) needs.
  • Complete all Incident Reports prior to the end of scheduled shift as required by Incident Policy and Procedure.
  • Maintain guests’ licensing requirements.
  • Oversee daily guest activities for guest(s) in the LBS.
  • Perform other duties as assigned.
